Doesn't always happen but on a server I had GM on we had our caster set bug out and any character who put it on was gone for good. Auto server crash if you logged in it.

that's probably just that server.

nah its pretty damn random with gems. on a server im GM on it took my pally friend 3 remade chars till we realised the meta gem (or was it red?) in the T7 healing mace was bugged. So its pretty random but yeah metas are the most common.

Whether you believe it or not, try not to mass meta gems. 15 meta gems socketed with the +healing on hit = GG. Horribly overpowered.

Iccy i think its good but i think i should add more stats because that has less stats then some rings well one or 2 rings
