[HELP] CANNOT ADD MY NEW CUSTOM ITEM {error log inside}

everytime i execute the SQL file of my new item i get this error. im using SQLyog btw.
can anyone please help me out?

EDIT:i also tried other weapons, i get errors on any new weapons i try and use, currently im using ascent 1662 with NCDB milestone2 DB.

nvm i fixed the error.
were it says name i added a 1 so i did "name1"
and it worked =]
