Vote page for website

I want a vote panel for my website where people can vote to get rewards, but I don't know how to add it to my website. I got a website when I downloaded my repack, I don't use wamp/xamp or anything. And I have a Ucoz website too. Is there a way to put up a vote page on one of the websites?
