to get there, go to user CP then under miscellaneous on the side bar click paid subscriptions

User CP > *Look at side bar on left* Miscellaneous > Paid Subscriptions > Select amount you'd like to donate > Fill everything required out, and so on.

Not sure, but if an Admin happens to be on the Shout Bawx at the time and they are feeling a little generous, you can ask if they'll change your name in the Bawx.

No so sure about the Name Change Request, because I have to many titles to know which one allows me to request a namechange :S (Member, Active Member, Contributor, Donator, Elite User, Moderator, Emulation Expert)
