need help. was banned after having my account hacked twice

hey, my account was hacked twice then blizz tempararily perma banned it until i could varify that my account was actually mine (which it was)

but i kinda screwed up...
just so you know, my the account is in my mom's name and she doesn't want me playing any more so....
i took a picture of her ID but there was the flash right where the name was, so blizz responded saying i needed a new picture. i used photo shop(it looks damn nice) but...when i saved the photo shop version i saved it as "edit-3.jpg(the name my photoshop recomended) so i saved it without thinking and submitted the photo and all the info required. they said that the ID could not be verified so they needed me to fill out form F:

and submit all the stuff back to them. But i'm having trouble figuring out what all i need to write. Like who is the notary and when does my "commission" end? if you guys could help me out by giving me an example of what "section F" should look like?

Why would you photoshop the ID? Just re-take the photo of the ID or scan it? Now your gonna have to get a notary, which is damn troublesome.

oh shit, just looked it up. i need to talk to a gov. official, WTF? that's BS. do you think if i just get a new pic of the ID they'll accept it?

A Notary Public is a certified person by the State to basically verify the identify of the person(s) signing a form. The seal that is used is state/county specific and their commission end time is like their expiration date.

is there any way around this? should i try submitting a new ID pic(not photoshopped)? or are they gonna require me to get a notary? because i'm just gonna get a new acc. if blizz is that way
