It is all done through the use of cell triangulation (If I remember correctly) and also by seeing which cell "node" your sim card is connected to... Once the phone is turned off and no signal is emitted.. bam. No tracking possible.
A phone (The actual hardware) can not be traced that I know of... as it requires the sim to send any signal. Although I do believe that the phone may send out a particular code when registering with a new cell each time (And it is this code that the networks can use to block a phone even with the sim changed).
If you are trying to track someone this would not be a way of doing it as I know of no public software that comes close to what you would need.....
