You can run LUA command mentioned here: MapID | WoWWiki | Fandom and then obtain result. This will require in-process approach.

If you want "passive" approach, you can see what is ID of map your character is standing in right now and then you can search WoW memory for that value. Remember all offsets. Then move to different map and again search for new map ID. See if some offsets are same in both tests. This is just an idea, I haven't tested it myself.
