Well considering all it's doing is sending a key press to the window that you select, I'm going to say yes, yes it is.

That's right. The current Warden implementation couldn't detect this as it is not modifying anything in the WoW process. It does have an opened handle to the WoW's window, but they don't detect/watch this at the moment and any legit program could open an handle to it.

Would it be possible to add a hotkey function to this program? so for example F5 starts spamming keys

what does the "Sleep" mean anyway? How is that used..I would guess its the interval between the sending..yes? If so how does it work..500 5 seconds..1000 10 seconds? Ooor what?

Delay between key presses. It's in milliseconds, so 5000 = 5 seconds.

Is this most updated version? Is there a way to the program detect a windowed WOW since I dont really like to use wow full screen, specially using 3 wow accounts. Ty in advance.

Agree, I have been using this for games for maybe 1.5 years now.
