This is very easy and effective

1. Goto <link removed because noobs thought i was advertising> or any other gold seller site and buy gold and give them you enemies name. ( yes u have to use real money )
2. Next wait about 3 hours and make a CSR appeal about that player buying gold.
3. laugh, laugh then laugh.
4. keep laughing, laugh some more and stop(maybe a couple more laughs to go with that)

I got this dude a 7 day ban hehe.

All this for $1.50

EVEN if you do that, you won't receive a notification on how they handle the whole situation.
This means you DON'T know what happens to this guy.

Those CSR ain't stupid and they'll first off talk to this guy...maybe give him a warning but that's it !

But seriously the only way i know is by hearing it from another person in his guild. Ill delete the link to prove i'm not advertising nuffin lol :S it was just and example of the site i used to get him banned. I'm not going to go into any deatail but he deserved this.

If you want to get someone out of a game just get your guild or friends report him for speedhacking and/or any hack you can see on youtube...
Tell them that you saw him doing XYZ, then you googled it and you saw the hack there too.

This method is way more effective!
Some guy spammed "LOL YOU SUCK!!!" to me and used the ignore bug to bypass my ignore (at this time you could just set yourself to anonym and there you go).
He did that for 3 whole days and got away with a warning.
After that I got pissed...and 2-3 days later my whole guild + people I knew reported him for bugging into walls, speedhacking and so on...BAM...he's gone now.
We all added him to our friendlist and no sign of him yet.
