Coding!

hey, do any sweet soul here got any small guide for a newb that should like to code a program? I want it to.. well. actually i want it so the information they fill in in the fields, getting send to my mail.
I just want a simple window like this :

ofc better lookcing and that. but something like that, but i will change the text and so on.
and if they press OK a like bar shows up that takes 20 min and go from 0 to 100% but it stops at like 50% and says a error message. but the things they filld in gets mailed to me.
if anyone got a form for this and which program i shuold use it in i will OFC +rep!
