I have been part of a modding community for a while and have seen this been pulled off hundreds of times. It is pretty easy to mod a Xbox 360, Modern Warfare 2 lobby. All you need is a jtag'd Xbox (look it up) and you're almost set.

You won't get banned if you get "boosted" through one of these mods (maybe in a rare case of obvious 1h played, level 10th prestige cases, yes), though only the modder will (the one with the jtag'd xbox).

Check out some modding communities for the Xbox-360, though this is very stressful and hard to get into one of these lobbies it is very fun, there are also more "fun" oriented mods which can get people infected and so other people can host them without having to need a jtag'd box in their possession.
