    Good book on C programming?

I know the title may not be as descriptive as it should be, but basically I'm looking for a good book which shows me how to turn thoughts into C, if that makes any sense.

What I mean is I can read pretty much any C source and understand what it does, but when I want to write a program myself I often find myself with just the basic skeleton code, knowing exactly what my program should do and how it should work, yet having no idea on how to turn the concept I have inside my head into a working, portable and modular program.

(English is not my first language, so the things I write might be a little confusing)

If you have no previous experience in programming I suggest starting with C# .NET then move your way down the C language chain... C++ ... C...

I have limited programming experience in x86 assembly, LISP, Scheme, C#, Python, C++, and C. I know the C language, and I can write programs in it. It's just that my programs are usually hard to modify or debug. I'm looking for a book which can provide me some insight into turning an idea you have in your head into a program which not only works correctly, but is also easy to modify, debug, and be read by other people.

Also, just to clarify, I write in C, not C++.

It's hard to come up with search terms for this. "C programming book" will get you books on the C language, and "C programming patterns" will get you books on C++ programming patterns. Once again, I know the language, I just want to learn how to make programs which are easy to modify and debug instead of programs which work but are hard to modify and debug.
